Case description:
Healthy 67-year-old female patient with bilateral maxillary molar mobility and infections, associated with posterior maxillary atrophy. Management required the use of Kits A and B to treat the left and right sides separately.
Left side treatment
-
- Observation of large retentional cysts present in the right and left sinuses. Bond Apatite fillings will be performed on the day of extractions to try to maintain a sinus floor.
- Healthier sinus situation, obtaining a sinus floor, the patient is ready for sinus fillings.
- Treatment plan
- 16-sections
- 17-sections
- After elevation, the two openings are clearly identified. Although slight bleeding is observed, the membrane remains intact, thus ensuring the possibility of grafting.
Bien qu’un léger saignement soit constaté, la membrane reste intacte, assurant ainsi la possibilité de greffer. - X-ray result taken 4 months after the operation
Conclusion on the left side:
The Bond Apatite filling, performed on the day of the extractions, allowed to maintain a minimum height of bone under the sinus, thus facilitating the use of Kit B.
Treatment on the right side
- Treatment plan
- The treatment plan provides for, in position 24, the use of KIT B; in position 26, the use of KIT A then KIT B; and in position 27, the use of KIT B.
- Sector to be operated
- In position 24 the B.Y.S at work
- An osteotome is used, the stop of which does not touch the crest, which requires the passage to the next B.Y.S in accordance with the protocol.
- After passing KIT B in positions 24 and 27 and placing the implants, we then move on to using KIT A in position 26.
- In position 26, after passing B.Y.1, the burr does not plunge correctly, hence the use of B.Y.2.
- B.Y.2 plunges successfully, providing a classic image that allows the osteotomes sequence to begin.
- Following the osteotomes, phase 3 consists of the Bond Apatite graft.
- The opening is closed and sutures are performed.
- The radiographic result, observed 4 months after the operation, confirms that KIT B has done its job satisfactorily.
- Radiographic result, before prosthesis
- Before/after comparison: The images show a clear improvement in bone anatomy.
Conclusion on the right side: :
The treatment, performed using Kit A, allowed for sinus elevation in a minimally invasive surgery, ensuring a safe intervention without secondary complications thanks to the use of suitable tools and materials.
